Step: 2.

Please fill the Admission form & attach all relevant documents as mentioned in the Admission Form. The documents required at the time of admission are as follows :

1. Attested Photocopy of 12th Passing Certificate
2. Attested Photocopy of 12th Marksheet
3. Attested Photocopy of 10th Passing Certificate
4. Attested Photocopy of 10th Marksheet
5. Attested photocopies of Undergraduate Course Marksheet ( for PG courses)
6. Internship Completion Certificate ( 6 months) for MPT
7. Attested photocopy of Undergraduate Degree Certificate/ Provisional Certificate ( for PG courses)
8. Character certificate from institution last attended.
9. Transfer Certificate from school/ Migration Certificate from University last attended.
10. 6 Passport size Photographs
11. 6 Stamp size photographs

1. Note:
   1. Incase some documents such as character certificate, degree certificate , etc are not available immediately, you may submit them within a stipulated time.
   2. The candidate may be required to produce above documents in original at the time of personal interview.

Step 4. Personal Interview:

All candidates will be called for personal interview/ counseling by telephone.

Step 5.

Selected candidates will be granted admission in the course opted for. After deposting the required fees within the stipulated date given at the time of personal interview. The fee can be deposited in the following ways

1. Demand Draft in favour of  I.T.S Institute of Health and Allied Sciences.
2. Online Bank A/c Transfer in the account of  I.T.S Institute of Health and Allied Sciences. A/c No <(Incase of Online Transfer please inform the Admission office immediately the details of transaction.)
3. By Cash at Cash Counter of Accounts Department,  I.T.S Institute of Health and Allied Sciences..

Please note

1. Mere filling up of the admission form does not guarantee admission. Candidates are advised to contact the Admission Office regarding the status of their admission.
2. In case of candidates whose qualifying examination results have not been declared the admission granted shall be provisional.
3. Management reserves the right to reject any admission form without specifying any reason.
4. All admission forms should be properly filled and signed by the candidates, parent/guardian.
5. Shortlisted candidates will be called for Personal Interview.
